The ReliaPet Pet Plan helps you communicate what to do with your pets in the event you are inaccessible. Have that peace of mind in knowing your pet will be cared for no matter what. Some things can draw us away from our pets unexpectedly: hospitalization, jail, death, and others. With this completed form in an easily findable location, your family and friends will know exactly how to care for your pet and what to do if something happens. Simply fill in the blanks to communicate your and your pet’s wishes to your family.

Some what’s and why’s:

Today’s date: Gives a reference points for other numbers needed in the document
Co-Habitability: This portion explains if your pet can co-habitate with other animals or species. If it is a prey animal, you can specify here (for those who might not know!) your pet needs to be kept separate from other pets. If your pet is social but must be kept from predator pets, you can simply write, “same species friends, away from predators”
Favorite things: What are your pet’s favorite enrichment items? Favorite treats? Knowing these individual quirks can come in handy when someone else is handling your critter.
Medical: This explains your pet’s health issues. Including contact information to your vets not only keeps your pet with someone experienced in their care, it gives the current caretakers a direct line to crucial information about your animal. If there is not enough space, you can attach more pages or write “read back of page” and add more info to the back. Knowing the location of your pet’s current meds can also be a often-overlooked lifesaver.
2nd and 3rd home: These are pre-discussed people/places your pet can go in the event you are unable to care for them for any length of time. I included a third home just in case the second home is unreachable, on vacation, or also unable to care for your pet for any reason. Make sure to talk to potential 2nd and 3rd homes before adding them here so you know they’re willing and prepared to take your pet on short notice. You can also add preferred shelters or sanctuaries if you don’t know or trust anyone to handle your pet. 
Other Info Field: This is really helpful if you need to add more information, or if something important to your animal does not have a spot on the form yet. You can add microchip ID numbers, bird leg band numbers, racehorse/mustang ID information, registration papers, breeder info (to ask the breeder questions about the animal), exercise schedules, where your cat likes to hide if they’re scared of people so the new/temporary caretaker can find them, and more!

This form is designed to be helpful to as many species as reasonably possible! Medically complicated pets, horses, chinchillas, lizards, frogs, cats, dogs, and many more! You never know what might happen.
